Tamplen Plastic Surgery (“we,” “us,” or “the practice”) operates this website at tamplenplasticsurgery.com. This policy explains what we collect through the website, why we collect it, who we share it with, and the choices you have.
This policy covers the website only. Health information that becomes part of your medical record is governed separately by our Notice of Privacy Practices under HIPAA. If you are already a patient, that notice is the one that applies to your care.
Our practice is located in the United States and this website is directed to patients in the United States.
1. Information you give us
When you submit the contact form, we collect the information you enter: your name, email address, phone number, the location you are interested in, how you heard about us, and whatever you write in the message field.
Please do not send detailed medical information through this website. Email and web forms are not encrypted end to end and are not a secure way to share health details. Once you become a patient, we will give you an appropriate way to share that information.
2. Information collected automatically
Like most websites, ours records technical information when you visit: your IP address, your browser and device type, the pages you view and when, the website that referred you, and the first page you landed on.
3. Advertising identifiers
When you reach this website by clicking an advertisement or a tagged link, the address may carry a code identifying that click. We store these:

We keep these in cookies set by our own website, and in your browsing session on our server, so that if you later submit the contact form we can tell which advertisement or link brought you to us. If you submit an enquiry, these values are stored alongside it.
4. Cookies
A cookie is a small file a website stores in your browser. We use these:

You can delete or block cookies in your browser settings. Blocking the session cookies will stop the contact form from working; blocking the others will not.

We do not sell any of this, and we do not use sensitive personal information to infer characteristics about you. We use it only for the purposes listed above, which are the purposes California law permits without a separate right to limit.
The procedure you enquire about is health information, so we treat the whole enquiry as sensitive. It is never included in what we send to advertising platforms — see section 7.
We do not use automated decision-making technology to make significant decisions about you, and we do not profile you for that purpose. Decisions about your care are made by your physician.
6. How we use this information
- To respond to your enquiry and arrange a consultation.
- To operate the website and protect it from spam and automated abuse.
- To measure which advertisements, pages and campaigns actually lead to enquiries, so we spend our advertising budget sensibly.
- To improve the content and design of the site.
- To meet legal obligations and to establish or defend legal claims.
We do not use automated decision-making or profiling to make decisions about your care.
7. Advertising measurement and offline conversions
We advertise on Google. To find out which advertisements lead to real consultations rather than just clicks, we send Google Ads a record when an enquiry results in a booking. That record contains:
- the click identifier from section 3,
- a label indicating a booking was made,
- the date and time,
- a value and currency,
- an internal reference number.
We do not include your name, email address, phone number, or the procedure you enquired about in what we send to Google.
If you do not want your visit used this way, see section 9.
8. Who we share information with
We do not sell your personal information for money.
We share information with service providers who help us run the website and the practice. They are permitted to use it only to provide those services:
- Google — Analytics, Ads, Tag Manager and web fonts (analytics and advertising measurement)
- Cloudflare — Turnstile, which checks that form submissions come from a person rather than a script
- Adobe — web fonts
- Fonticons — Font Awesome icons
- UserWay — the accessibility widget described in our Accessibility Statement
- Our hosting provider — servers located in the United States
- Our web and marketing agency — site maintenance and advertising management under contract
We may also disclose information where the law requires it, in response to valid legal process, or where necessary to protect the rights and safety of our patients, our staff or the public.
A note on the word “sharing.” Sending advertising identifiers to Google, Meta or Microsoft as described above may count as “sharing” personal information for cross-context behavioral advertising under California law, even though no money changes hands. Section 9 explains how to opt out.
9. Your choices
Global Privacy Control. If your browser or extension sends the Global Privacy Control signal, we treat it as an opt-out request. We will not store advertising identifiers for your visit, and we will discard any we already stored during that visit. You do not need to contact us.
Browser controls. You can block or delete cookies in your browser settings.
Google. You can install Google’s Analytics opt-out browser add-on and adjust your Google Ads settings.
Ask us. Write to [email protected] and we will opt you out, or access, correct or delete what we hold from your website enquiry.
10. California privacy rights
If you live in California, you may ask us to:
- tell you what personal information we have collected about you, where it came from, why we collected it, and who we shared it with;
- delete personal information we collected from you;
- correct personal information that is inaccurate;
- stop sharing your personal information for cross-context behavioral advertising;
- limit our use of sensitive personal information.
You may also ask us for information we collected about you more than 12 months before your request, and we will provide it unless doing so proves impossible or would take a disproportionate effort. We will tell you if that is the case.
We will not deny you services, charge you a different price, or give you a lower quality of service because you exercised these rights.
To make a request, email [email protected] or call any of our offices. We will ask for enough information to confirm your identity before we act, and we will use what you give us only for that purpose. You may use an authorized agent, in which case we will ask for proof of their authority.
Medical information held in your patient record is governed by HIPAA and the California Confidentiality of Medical Information Act rather than by these website rights. Requests about your medical record should go to the practice directly.
11. How long we keep information
- Advertising click identifiers are automatically cleared 90 days after the enquiry. After that they can no longer be linked to an advertising click.
- Campaign labels and referring pages are kept longer. They describe a campaign, not an individual click.
- Enquiries are kept for as long as we need them to respond to you and to keep proper business records.
- Analytics data is kept according to the retention period set in Google Analytics.
12. Children
This website is not directed to children under 13, and we do not knowingly collect their information through it. We do not knowingly sell or share the personal information of anyone under 16. If you believe a child has submitted information to us, please contact us and we will delete it.
13. Security
The site is served over an encrypted connection, enquiries are stored on access-controlled servers in the United States, and only staff who need the information can see it. No website or method of transmission is completely secure, so we cannot guarantee absolute security.
14. Links to other sites
This site links to other websites, including hotels, restaurants and professional organizations. We are not responsible for their content or their privacy practices. Please read their policies before giving them your information.
15. Changes to this policy
We may update this policy. When we do, we will change the date at the top of this page and post the new version here.
